JCI quotes an average of 46,000 US dollars a year to hold the accreditation, plus travel and other costs, and the fee is set from the services a hospital offers and its average daily census. That structure is not a scandal; it is how nearly every voluntary accreditation on earth works, and saying it out loud is the only way a reader can size what the mark means.\n\nThe survey is scheduled. JCI's own American surveys are unannounced; its international ones \"still take place at a time known in advance by the hospitals being surveyed, often after considerable preparation by those hospitals\". A reader who takes the mark as a snapshot of any ordinary Tuesday is reading it wrong.\n\n*Inference —* what the accreditation reliably certifies is that a named external body inspected named systems on a named date and found them conforming. It does not rank hospitals against each other, does not measure surgical outcomes, and does not survive the three years by itself. This directory therefore counts accreditations and never weighs them.",
