They are written by different parties for different purposes, and the gap between them is where an unplanned bill falls.",
  "story": "On the provider side the list is often longer than the inclusions. The knee package this site reads at one Bangkok hospital covers three hours of theatre, three nights on the ward, surgeon, assistant and anaesthesiologist — and excludes the implant, the first consultation and the follow-up visits, nights beyond three, any intensive care, blood transfusion, laboratory and imaging before or after surgery, take-home medication, and complex or emergency cases. A joint package that excludes the implant is quoting the operation, not the joint.\n\nOn the insurance side the NHS states the general position for its own readers plainly: travel insurance \"does not normally cover if things go wrong in surgery\". The UK government's travel insurance guidance is written around emergencies and lists repatriation, emergency transport and getting home after treatment when the original ticket cannot be used; it also warns that EHIC and GHIC \"are not alternatives to travel insurance as they do not cover any private medical healthcare costs, repatriation or additional costs\".\n\nThe CDC Yellow Book puts the same duty on the traveller in its own terms: check what the domestic plan covers outside the country, and consider supplemental cover \"including medical evacuation insurance\".\n\n*Inference —* the structural trap is that a planned operation is not an emergency, and most travel policies are emergency instruments. A complication of a procedure the traveller flew out to have can be a genuine emergency in the ward and an excluded event on the policy at the same time.",
