  "story": "The recipient side has been measured, and the measurements agree. A transplant centre in Riyadh compared 60 patients who had been transplanted abroad against 254 transplanted locally and followed at the same hospital between January 2015 and December 2018. Delayed graft function ran at 18.3% against 6.3%. Acute rejection ran at 40% against 7.9%. Graft failure at three years ran at 11.7% against 0.8%. Post-transplant infection and urological complications were both higher. The travelling group were younger; the two groups were otherwise similar on sex, body mass index, diabetes and hypertension.\n\nA 2021 review of liver transplantation reached the same conclusion for that organ: higher post-transplant complications and higher mortality than domestic transplantation, with improper pre- and post-transplant infectious screening, inadequate prophylaxis against opportunistic infection, and loss to follow-up named as the mechanisms.\n\nThe donor side is the reason this record is filed as an ethical risk rather than a clinical one. The Declaration of Istanbul, first agreed in 2008 and revised in 2018, is the instrument the field uses to separate three things that get collapsed together: a patient travelling for a transplant, the sale of organs, and the trafficking of people for their organs. Only the first is lawful anywhere.\n\n*Inference —* the recipient figures above understate the harm in one direction and overstate it in another. They count the patients who came back and presented to a follow-up clinic, so a recipient who died abroad is not in them; and they are single-centre, so they carry the referral pattern of the hospital that published them.",

The parts that go wrong are identifiable in advance and none of them is surgical: donor infectious screening, tissue matching, the record of what was given and when, and whether anyone at home has agreed to take the follow-up before the flight is booked.",
