  "story": "Read one and the shape is plain. Bumrungrad's total knee replacement package, JOINT04, read on 16 September 2026, buys three hours of theatre, the nursing and surgical team within those three hours, three nights on the surgical ward, equipment and supplies, medications related to the procedure, the surgeon, an assistant surgeon if needed, and the anaesthesiologist. Its exclusion list is longer: the implant, the first consultation and the follow-up visits, specialist consultations, nights beyond three and any intensive care, blood transfusion, laboratory and imaging before or after surgery, take-home medication, outpatient charges before admission, and complex or emergency cases. The package name itself carries the word — exclude implant.\n\nA package can also be the instrument that hides a unit. Estenove in Istanbul prices hair restoration at 3,395, 4,595 and 6,595 euro for what it calls the maximum number of grafts, and states that it does not price per graft. One seller's package removes the unit of comparison; another seller's package removes the largest line on the bill.\n\n*Inference —* a package price is therefore comparable only against a package with the same list. A joint package that excludes the implant and one that includes it are two different objects wearing the same word, and the gap between them is not a discount.",
