Said as one word on a menu board — 'mac and cheese' — and counted as a vegetable on a meat-and-three line.
Macaroni in a cheese sauce, most often cheddar, baked in a dish until the top browns. On a Carolina barbecue buffet it sits with the vegetables and is taken as one, cut in squares from a hotel pan rather than spooned; a stovetop version bound with a white sauce is the other common form. In the Lowcountry and in Black Southern kitchens the baked, custard-set kind is often called macaroni pie.

The dish reaches the South through the first cookbook written here. Elizabeth Raffald printed a baked macaroni with cream, butter and Parmesan in England in 1769; one account has James Hemings, enslaved by Thomas Jefferson and trained as a cook in France, bringing the method to the United States, and Jefferson served 'a pie called macaroni' at a state dinner in 1802. Mary Randolph's The Virginia House-Wife of 1824 carries a recipe under the name macaroni and cheese: three things, macaroni, cheese and butter, layered and baked in a hot oven. Mrs. A. P. Hill's Southern cookbook of 1867 gives the same layering and then a custard version with three beaten eggs poured over. Factory macaroni and factory cheese put it within reach of every household, and by then it had stopped being a rich person's dish. On a Carolina barbecue menu it is a late arrival and a well-established one: at Luella's in Asheville it is on the board because the daughter the restaurant is half named for likes it.

Macaroni is boiled in salted water until just tender and drained. For the baked kind it is layered in a dish with grated cheddar, salt and pieces of butter, covered with milk or cream, and baked until the top browns; for the custard kind beaten eggs and evaporated or whole milk are poured over the layers so the dish sets and can be cut. A stovetop version makes a Mornay — a white sauce with cheese melted into it — and stirs the macaroni through. At a pit it is baked in a hotel pan in the morning and held warm on the line.

- One-half small sized package of macaroni,
- One-third pound cheese.
- One cup of milk.
- Butter the size of a walnut.
- One-half cup of bread crumbs.

- 4 cups (1 L / 1.1 quart) milk, heated
