The church has burnt, fallen and been rebuilt since 1588: destroyed by Limahong's men in 1574, burnt in 1603, damaged by the British in 1762, burnt in 1791 with the image spared, thrown down by the earthquake of 1863, burnt again in 1929. The present reinforced-concrete building, by the architect Juan Nakpil, was finished in 1935 and expanded in 1984-86. Pope John Paul II raised it to a minor basilica in 1987; it was declared a national shrine in December 2023.

The devotion is enormous. The feast of the Traslación on 9 January re-enacts the image's transfer from Intramuros, and 6.5 million devotees took part in 2024. Every Friday is Quiapo Day, with a novena and thousands in attendance; devotees climb a narrow stair to kiss the statue's foot or wipe it with a handkerchief they bring each visit. Men who carry the image walk under a panata, a vow. In October 2024 the Archbishop of Manila decreed the church's name changed to the Minor Basilica and National Shrine of Jesus Nazareno, dropping the usual English name to shift attention from the image's colour to the name of Christ.

The plaza trade is the other half. Agimat — anting-anting in its folklorised form — is the Filipino word for an amulet, and the practice that goes with it: most bear Latin inscriptions, most come with a small book of incantations to be read on Good Friday. Agimat merchants set up near churches, in the courtyard or the marketplace alongside, and Quiapo is the district the encyclopedia names when it names one. Filipino fighters wore anting-anting against the Spanish and the Americans; Macario Sakay wore a vest of religious images and Latin phrases.

The church has not been quiet about what else the stalls sell. Published accounts record peddlers of unsafe abortifacients and untested herbal remedies working the vicinity, and the Archdiocese of Manila condemning practices connected with them; Cardinal Gaudencio Rosales issued canonical excommunications in relation to abortion near the shrine. Abortion is illegal in the Philippines, and the accounts tie the trade to people who cannot afford a surgical procedure.
