The horn is worn against the malocchio, the evil eye, and against bad luck generally, and historically for fertility and virility. It is a regional amulet before it is an Italian one: most often worn by men in southern Italy, above all in Campania, then Lazio, Apulia and Basilicata, and less in Sicily and Calabria.
What the horn is a horn of is an open question. Cornicelli are thought to be modelled on an eland horn. The red ones look like a chili pepper, and dried chilies are hung in some southern Italian houses against the malocchio — but chilies reached Europe only after the Columbian exchange, so the resemblance is younger than the shape. The classical lines pull in several directions at once: the cornucopia broken from a goat by Zeus; coral sacred to Venus; silver sacred to Luna; the phallic outline and the red pointing to Priapus; the horn itself to Pan.
What the harm is said to reach is specific. The malocchio is described as harming nursing mothers and their babies, fruiting trees, milking animals and the sperm of men — the forces of generation, in that order.
The horn moved from the animal to the machine without changing shape. Cornicelli hang from rear-view mirrors now, which the sources trace to the older custom of hanging them on draft horses.
Inference — this atlas files the horn beside the eye rather than as an unrelated sign, because the logic its carriers describe is the same interception: a thing conspicuous enough to take the look meant for somebody else.

Made in gold and silver for jewellers, and in plastic by the tray for the tourist trade. San Gregorio Armeno in Naples sells both, on the same street, a few metres apart.
