This one has a date, a witness and a goldsmith, which almost nothing else in this atlas does.
Catherine Labouré, a novice with the Daughters of Charity in the rue du Bac in Paris, said that on 18 July 1830 a child's voice woke her and led her to the convent chapel, where the Virgin Mary told her: "God wishes to charge you with a mission [...] You will be contradicted, but do not fear, you will have grace." On 27 November the same year, she said, Mary returned during her evening meditations, standing on a globe inside an oval frame, wearing rings that shed light — the rays, Mary told her, were "the graces I shed upon those who ask for them", and the gems that stayed dark "are the graces for which souls forget to ask". The frame turned, and the reverse design appeared. Then the instruction, which is the whole point of the object: have a medal struck after this model, and "all who wear them will receive great graces".
Her priest watched her ordinary daily behaviour for two years before taking it to Archbishop Hyacinthe-Louis de Quélen of Paris. Approval came; the goldsmith Adrien Vachette made the die; the first 1,500 medals were minted on 30 June 1832. Then the arithmetic gets strange. The Daughters of Charity distributed them during a deadly cholera pandemic, cures were reported, and the public renamed the object. Over 500,000 had been distributed by 1834. By 1835, more than a million worldwide. By 1839, over ten million. By Labouré's death in 1876, more than a billion had been produced. Inference — a devotional object that reached a billion within a single lifetime is an industrial fact as much as a religious one, and the cholera years are where the curve bends.
On the Church's side the object sits inside a category. The Miraculous Medal is a sacramental — by the Catholic Church's definition a sacred sign that works "through the intercession of the Church", which prepares a person to receive grace and disposes them to cooperate with it, and which does not act by itself the way a sacrament does. The Enchiridion Indulgentiarum of 2004 grants a partial indulgence to the faithful who use a crucifix, a crown, a scapular or a medal blessed by a priest. The 2001 Directory on Popular Piety and the Liturgy lists Marian medals among approved practices while holding that popular piety "may never incorporate rites permeated by magic, superstition, animism, vendettas or sexual connotations". Inference — the medal's own promise, struck into it, is that wearing it brings graces; the doctrine is that the wearing disposes rather than delivers. Both sentences are on the record, they are not the same sentence, and this atlas does not decide between them.
One wearer's account is famous enough to be in the liturgy. In 1842 Marie-Alphonse Ratisbonne, a Jewish man, took a bet to wear the medal on a trip to Rome, entered the basilica of Sant'Andrea delle Fratte, and reported a vision of the Virgin exactly as the medal depicts her. He converted, became a Jesuit, and the event has its own canonical inquiry behind it and a place in the Feast of the Miraculous Medal. Maximilian Kolbe, founding the Militia of the Immaculate in 1917 under this Virgin's patronage, handed the medals out and called them his "ammunition".

Struck from the 1832 die design by religious-goods manufacturers worldwide. Bought or given; the giving is the commoner route, and the Daughters of Charity have handed them out free since the first batch. A priest may bless one, which is what attaches the indulgence; nothing requires it. Worn at the neck under clothing more often than over it. Never retired — a lost one is simply replaced.
