A penis is built from a flap of the patient's own skin, fat and nerve — most often the forearm, otherwise the thigh, groin or back — transferred to the pubis and connected to blood vessels and nerves there. Where urethral lengthening is done, a tube is made inside the flap so that urine passes through the new organ; that is the part that leaks, narrows and needs repair most often. Rigidity, where it is wanted, comes later from an implanted prosthesis, and testicular prostheses later still. The work is staged across separate admissions and usually separate years.

The clearest published statement about what this operation costs is a Bangkok hospital's list, and it is published one stage at a time. Yanhee Hospital prices a total phalloplasty at 426,000 baht with fourteen nights attached; before it, urethral prefabrication on the forearm at 97,700 with one night, and vaginal closure at 199,000 with five; after it, a coronoplasty at 96,000, a penile prosthesis at 99,000 and a scrotal prosthesis at 96,000, the last two stated to exclude the implants themselves. Inference — the six rows add to 1,013,700 baht before any implant, which is the number a reader comparing this with a single headline price is actually looking for, and no page publishes it.

The same table carries a line that is rarer than the prices. Vaginal closure is quoted at 199,000 baht where the hysterectomy was done at Yanhee and 228,800 where it was done, in the table's own words, at "Other Places". A staged operation split across two countries costs more at the second one, and here somebody has put a figure on it.

In the United States the Gender Confirmation Center estimates phalloplasty at 35,000 to 50,000 dollars and then says something a price page rarely says: the operation is "not available to cash-pay patients" and needs insurance pre-approval, while metoidioplasty, vaginectomy and groin flap phalloplasty are available without it. That is a surgical practice declining to sell the largest operation on its own list for cash. In London, the private list that publishes guide prices for chest surgery and for vaginoplasty has no phalloplasty or metoidioplasty row on it at all.

Inference — those two absences point the same way. This is the operation in the gender-affirming set with the most stages, the most revisions and the longest tail of complications, and the pages that would otherwise quote a price are the pages that stop.

General anaesthesia, long operations, and an admission per stage. The flap is raised with its artery, vein and sensory nerve, moved, and plumbed in under a microscope; the donor site is grafted and leaves a permanent scar the size of the flap. Where the urethra is lengthened, it is usually prefabricated in a separate earlier operation. Hospital stays on the one published table run from one night for the prefabrication to fourteen for the phalloplasty itself. Candidacy conditions are published by the clinics rather than imposed by them alone: one Bangkok hospital lists a minimum age of 20, at least a year of testosterone, a year living in the acquired gender, psychiatric clearance, and mastectomy and hysterectomy completed at least six months earlier.

Two countries' provider pages carrying a phalloplasty price were read on 17 September 2026 — a Thai staged list and a US practice's estimate that comes with a refusal to take cash for it. Between those two numbers sit a different currency, a different number of stages, a different definition of what is included, and the implants, which the Thai list excludes by name.
