Day-surgery guidance calls the missing person a responsible adult. The booking calls it a hotel transfer.

Discharge from a hospital abroad into a hotel room rather than a household. Two instruments this project has read require an adult at the other end of a general anaesthetic, and both were drafted for a patient going home. Nobody counts how many people who travel for treatment are discharged without one.

The requirement is written down in plain terms. The Royal College of Anaesthetists, in Chapter 6 of its Guidelines for the Provision of Anaesthesia Services, published 31 March 2021, recommends at 5.25 that "Following procedures performed under general or regional anaesthesia, a responsible adult should escort the patient home and provide support for the first 24 hours after surgery", qualified by the observation that "A carer at home may not be essential if there has been good recovery after brief or non-invasive procedures and where any postoperative haemorrhage is likely to be obvious and controllable with simple pressure." The recommendation is graded C and strong. Two lines later, at 5.26: "Transport home should be by private car or taxi; public transport is not normally appropriate." At 5.24, a 24-hour telephone number "should ideally be to an acute surgical area and should not be an answer phone".

The United States puts it in a condition of coverage rather than a guideline. 42 CFR 416.52(c)(3) requires an ambulatory surgical center to "Ensure all patients are discharged in the company of a responsible adult, except those patients exempted by the attending physician" - a rule in force since 18 November 2008 and amended on 30 September 2019, binding on facilities billing Medicare.

Inference - read together, the two describe a household. A car at the door, a person in the building overnight, a telephone answered by someone who can send an ambulance. A patient recovering in a hotel three time zones from home has a taxi rank, a front desk and a clinic's daytime line, and the hotel is not a party to the discharge.

The gap between the two is unmeasured. No ministry, accreditor or registry publishes how many patients treated abroad travel with a companion, how many are discharged to a hotel, or what happens to those who arrive alone. This site's pathway record for the companion reaches the same wall from the other side: the only hard number attached to that step anywhere is an immigration quota.

What is published instead is the shape of the package. The Bangkok knee package this site reads names three nights in the surgical ward and then stops; nights beyond the third are billed at the hospital's rates, and nothing after discharge is inside the price at all.

How it works Trade practice

The lifting is the part that does not appear in any document. Somebody gets a post-operative body from a wheelchair at the hospital door into a taxi, and out of it at the other end. Somebody watches the dressing overnight, times the analgesia, decides at 3am whether a temperature is the operation or a complication, and finds the hospital number. Where no such person travelled, the roles fall to hotel staff with no clinical duty, to a facilitator's coordinator working office hours, or to nobody.

Trade practice - the hotel room, the recovery apartment and the escorted transfer are sold as convenience rather than as care, and the packages this project has read do not describe who provides nursing in them or on what hours. Where a hospital offers a nurse or a serviced recovery suite, that is a separate purchase with its own price.

As of 17 September 2026 the named guidance and the named regulation stand, and no published count exists of medical travellers discharged without a responsible adult. What can be stated is narrower: two health systems require a person at the other end after an anaesthetic, and the accommodation sold with a treatment package is not required by either of them to supply one.
