Two trades share one country and they have almost nothing in common. Crush and Chikanda, writing in Social Science & Medicine in 2015, made the case that movement to South Africa from within the region is larger, in numbers and in money, than movement from the global North — and that the second is what gets studied while the first is where the patients are. Patients from Zimbabwe, Zambia, Botswana, Namibia, Mozambique and further north arrive for oncology, orthopaedics, fertility and cardiac work, because those services exist in Johannesburg, Pretoria and Cape Town and do not exist reliably at home.
The European half is priced, packaged and advertised. Breast augmentation, liposuction, nose reshaping, facelifts and abdominoplasty are the operations named most often, alongside dentistry. Wesgro, the Western Cape's own trade and investment agency, recorded the province's share of arrivals travelling for medical reasons rising from 0.8 per cent in 2015 to 0.9 per cent in 2016, and named the United Kingdom and the Netherlands as its top source markets, with German visitors coming largely for dentistry.
Inference — the survey share is the better number and it is small. South African Tourism asks arriving visitors their main purpose of visit; 1.8 per cent of them said medical in 2016, down from 2.3 per cent in 2014. That is a measured share of a measured population, and it sits underneath every larger figure quoted for this country without one.
How it works Trade practice
The European route is an agency booking against a package quoted in rand, with the exchange rate doing much of the selling and a recovery stay attached. The regional route is a referral, often self-funded, sometimes by a national health ministry buying a service its own hospitals cannot provide, and it arrives at a private hospital group or a public tertiary centre rather than at a cosmetic clinic. Wesgro's own interviewee, Faith Cartwright of Medical Tourism South Africa, said arrivals here tend not to buy treatment packages at all, being wary of undisclosed costs inside them.
