A federation of seven emirates that buys care abroad for its own citizens and sells care to visitors at the same time. Dubai licenses and counts an inbound trade of its own and publishes an annual number for it; a separate state programme, with its own bylaw, pays for Emiratis to be treated in Europe, the United States and Asia, and pays their escorts a daily allowance while they are there. Health regulation is emirate-level, not federal, so a clinic in Dubai, one in Abu Dhabi and one in Sharjah answer to three different licensing bodies.

Dubai Healthcare City opened in 2002 as a free zone with its own clinical regulator, licensing providers and professionals inside its boundary rather than leaving them to the emirate's general health authority. Its first phase covers 4.1 million square feet of hospitals and medical education; a second, dedicated to wellness, is under development. A free zone with a regulator attached is the structural answer to the problem every new destination has, which is that a foreign patient does not know what a local licence means.

The inbound number the emirate publishes is large and specific. The Dubai Health Authority reported "more than 691,000 health tourists" in 2023, spending AED 1.03 billion directly, against 674,000 and AED 992 million the year before. Dentistry accounted for 29 per cent of what they came for, dermatology 27 per cent and gynaecology 13 per cent. Inference — those three shares are the tell. A count led by teeth and skin is counting short outpatient visits by people already in the country, not tertiary admissions flown in for surgery, and the release does not publish the rule that separates a visiting patient from a resident one.

The outbound side is documented in a way the inbound side is not. Administrative Resolution No. 14 of 2017 sets out who the Dubai Health Authority will send abroad and what it will pay: the treatment, the physicians, the medicines and devices, interpreting, air ambulance where needed, and repatriation of the body if the patient dies. One escort travels with an adult patient; two accompany a child, or a patient who is elderly, paralysed, comatose or disabled. The schedule at the back sets the daily allowance by destination — €150 a day for the patient and €150 for the first escort in European countries, US$150 in the United States, AED 300 in India, Thailand and Arab countries.

Inbound, a visitor books a private clinic directly or through a licensed facilitator, pays cash or against an international policy, and is treated under an emirate licence: the Dubai Health Authority, the Abu Dhabi Department of Health, the federal ministry for the northern emirates, or Dubai Healthcare City's own regulator inside the free zone. Outbound, the route is administrative rather than commercial: a medical report from a government or accredited private hospital, a committee recommendation, then an approval that carries the escort, the flights and the per-diem with it.

As of September 2026 the most recent inbound figure Dubai has published is the 2023 one. No comparable published count exists for the number of Emiratis sent abroad under the state programme, so the two flows cannot be set against each other on like evidence. By headcount the inbound number is far the larger; by cost per patient the outbound one is not close, because a state does not fly a citizen to Germany for a filling.

The 691,000 figure is the Dubai Health Authority's count for one emirate, not a national total, and this record does not extend it to the federation.
