Follicular units are harvested as in follicular unit extraction, then loaded one at a time into a hollow implanter pen that both makes the recipient channel and delivers the graft in a single movement, rather than the two-stage method of cutting slits first and placing grafts into them. The harvest, the anaesthetic, the day and the donor limits are the same as FUE. What changes is the instrument, the number of staff hours, and the price tier.

DHI is a brand-linked technique rather than a separate operation, and the record says so because the price pages do not. DHI Medical Group's own Dubai page describes a patented implanter pen that allows surgeons to directly implant hair follicles without the need for creating incisions beforehand, and claims more precise control over the depth, direction and angle of each implanted follicle, a no-shave option, and a higher survival rate of grafts because follicles spend less time outside the body. Those are the seller's claims, published by the seller.

The pricing is where the distinction shows. Estenove in Istanbul quotes the same package prices - 3,395, 4,595 and 6,595 euro - for FUE and DHI alike, which is one clinic's answer to whether the pen is a different product. DHI Medical Group in Dubai publishes a ladder instead: about 9,999 dirham for minor procedures with fewer grafts, 12,000 to 18,000 for moderate hair loss, and 35,000 or more for extensive work.

Trade practice -- the argument between the two techniques is conducted almost entirely in clinic marketing. Whether the pen improves graft survival, and by how much, is not settled by any trial that this record could find published; what is documented is that it changes how many staff are needed and how long the session takes, and therefore what the session costs.

The question the International Society of Hair Restoration Surgery raises about hair surgery generally applies here with particular force, because the pen is often the part of the operation handed to technicians. Its position statement holds that donor hair harvesting, hairline design and recipient site creation should be done by a properly trained and licensed physician or physician extender; in the pen technique, recipient site creation and graft placement are the same movement.

Local anaesthetic, a full day, no ward. The scalp may be left unshaved, which is the feature most often sold with the technique and the one that lengthens the session. Loading pens is slow, so a DHI session usually involves more assistants than an FUE session of the same size, or fewer grafts in the same time. Aftercare, shedding and the twelve-to-eighteen-month judgement date are identical to FUE.

Two published prices sit on this record, from Turkiye and the United Arab Emirates, read in September 2026. One clinic prices the pen the same as the punch; the other prices by extent of hair loss. Neither publishes a graft count with the price.
