Anaesthetists say ASA 1 through 6, with an E for emergency. Nobody says it to a patient, and it decides whether the patient is operated on that morning.
A six-point scale describing how well a patient is before an operation, from ASA I, healthy, to ASA VI, declared brain-dead with organs being removed for donation, with an E appended where the surgery is an emergency. It is the shorthand every anaesthetic record in the world carries.

The story Cited
The scale ranges from a healthy patient through mild systemic disease, severe systemic disease, severe systemic disease that is a constant threat to life, and a moribund patient not expected to survive without the operation, to the brain-dead donor. It was introduced in 1941 and has been revised repeatedly since; the current table was approved on 15 October 2014 and last amended on 13 December 2020.
Inference — the grade is where a remote quotation and a real patient meet. A package price, a stated length of stay and a fitness-to-fly date all assume a low grade. The grade is assigned face to face, after the history, usually on the morning of surgery, by a doctor who has not seen the quotation. A patient graded higher than the package assumed is the commonest way a fixed price stops being fixed.
How it works Inference
Three things follow from the grade: whether the operation proceeds as planned, whether it needs a higher level of monitoring or a bed in intensive care, and how long the patient stays. Intensive care sits outside almost every published package.
