The sector names itself by the traveller's motive, and then counts everybody whose motive was partial.

Travel undertaken to maintain or improve wellbeing rather than to treat a condition. The body that publishes the sector's figures, the Global Wellness Institute, defines it as "travel associated with the pursuit of maintaining or enhancing one's personal wellbeing". It is not medicine, it is routinely counted beside medicine, and the two overlap wherever a state uses the wider health word.

Root. wellness is 1650s, "state of being in good health", from well as an adverb plus the suffix -ness, on the Proto-Indo-European root meaning to wish or will. It lay almost unused for three centuries. Halbert L. Dunn revived it in its modern sense in 1959, in "High-level wellness for man and society" in the American Journal of Public Health, and in the book High Level Wellness in 1961; Donald Ardell carried it to a general readership in 1977. The word has never meant a diagnosis or a treatment, which is what recommends it commercially.

The story Cited

The counting rule is published, which is more than most figures in this field manage. The institute splits travellers in two and counts both: a primary traveller, "whose trip or destination choice is primarily motivated by wellness", and a secondary one, who "seeks to maintain wellness while traveling or participates in wellness experiences while taking any type of trip for leisure or business".

It also counts the whole trip rather than the wellness part of it, and says so: "In addition to wellness experiences, all wellness tourists need transportation, food and lodging, and they will likely seek out shopping or entertainment. All of these businesses—whether they are wellness-specific or not—benefit from wellness tourism and are part of the wellness tourism economy." Against that rule the institute puts the sector at 894 billion US dollars in 2024; its first report, in 2013, gave 439 billion for 2012.

The institute draws the line this site depends on and disowns the conflation itself: wellness travel sits on the proactive side of a health continuum, and travel for surgery or dentistry on the side of illness and injury, with "health tourism" named as the catch-all that causes the confusion.

Inference — a total built on secondary motive and whole-trip spending is not a count of anything a hospital would recognise. It is how a national figure can run to millions of arrivals while the operating lists stay short.
