The Central American destination that organised itself earliest. Dentistry and cosmetic surgery are the volume trades, sold to North Americans on a two-to-three-hour flight from Houston or Miami, and the country's private hospitals in San José take the complex work. Costa Rica also runs a universal public system, the Caja, which the visiting patient does not enter and is not counted by.

The organising body is PROMED, the Consejo para la Promoción Internacional de la Medicina de Costa Rica — a national council that certifies and lists providers for international patients, and the only Latin American body of its kind on this site's recognizer list. Inference — a council that certifies its own members is a trade association with an inspection attached, and the certificate says a provider paid for and passed that inspection, not that the outcome of an operation there is better. On 16 September 2026 the promedcostarica.org address answered with a redirect to the Cámara Costarricense de la Salud, a health-sector chamber in San José; whether the council has been folded into the chamber or has only moved its web address is not something this record could establish, and the PROMED entry is flagged for verification because of it.

The hospital side is older than the trade. Hospital Clínica Bíblica in San José describes itself as "Costa Rica's longest-running private hospital", at "96 years", and carries accreditation by Joint Commission International — a US-derived hospital standard, run on a three-year cycle, which the applicant pays for.

Trade practice — dentistry is the reason most patients come, and dentistry is the part with the least public data anywhere. A country can have a certifying council, a JCI-accredited hospital and no register at all of who fitted an implant in a shopfront in Escazú.

Trade practice — the usual Costa Rican arrangement is the co-ordinator: a clinic or a facilitator quotes the treatment, the airport transfer and the recovery hotel as one line, and books the consultation for the morning after landing. Long dental courses are staged across two trips with a temporary fitted in between, which is why the flight cost appears twice in the real total and once in the quoted one. Payment is a deposit to hold the surgical date and the balance on arrival; North American dental insurance rarely pays any of it.

As of September 2026 this record carries no national count of foreign patients: the Costa Rican figure most often quoted in the trade press comes from market summaries whose method this project could not read, and it is left out rather than repeated. What is carried is narrower — the accreditation Clínica Bíblica states, and PROMED's own role as the certifying council.
