Clinics say All-on-4 and mean one jaw. Price lists in three countries print it the same way, which is what a trademark does: it standardises the word without standardising the bill.

Four implants are placed in one jaw, the back pair angled forward to avoid the sinus or the nerve, and a fixed bridge of ten to fourteen teeth is screwed onto them — often a temporary one within about a day, with the definitive bridge months later. Root. Not a derivation but a trade name, coined in 2003. Maló, Rangert and Nobre published the "All-on-Four" immediate-function concept with Brånemark System implants for completely edentulous mandibles in Clinical Implant Dentistry and Related Research, 2003;5 Suppl 1:2-9 — 44 patients, 176 immediately loaded implants, a fixed complete-arch prosthesis in function within two hours. Nobel Biocare lists All-on-4 among its registered trademarks, with "treatment concept" given as the generic term.

The name is the first thing to read on the bill. All-on-4 is a registered trademark of Nobel Biocare, used for products and services alike, and it names a technique — four implants, the back two tilted, loaded at once — rather than a material or a standard.

The three price lists read for this record all quote it per arch and all quote it differently. Sani Dental Group in Los Algodones prints three versions of the same trademark side by side: the system with an acrylic hybrid fixed denture at $8,110, the same thing on Nobel implants at $8,950 and on Straumann implants at $9,710, each marked as one arch, each with a "US COST" column beside it — $25,000, $27,000, $30,000 — that is the clinic's own comparison and carries no source. Medicare Dental Clinic in Budapest splits the bill the way it splits every other: All-on-4 zirconium at €2,503 of treatment plus €1,668 of laboratory fee, All-on-4 metal-ceramic at €2,265 plus €1,509, with the six-implant versions above them. Bangkok International Dental Center quotes All-on-4 and ProArch hybrid bridges from 290,000 baht and, on the same page, dental implants plus fixed bridges from 140,000 plus 250,000 — a second route to the same jaw at nearly twice the price.

Inference — the gap between the Budapest figure and the Mexican one is not a gap between two countries' surgery. It is a gap between two ways of writing a price list: Budapest names the laboratory fee and leaves the implants to their own lines, Los Algodones bundles implants, abutments and bridge into one number. A reader comparing €4,171 with $8,110 is comparing a subtotal with a total.

How it works Trade practice

Assessment and a CT scan, extraction of whatever teeth remain, four implants placed under local anaesthetic with or without sedation, and a provisional bridge fitted the same day or the next. Months of healing follow, then the definitive bridge. For a patient who has flown in, that is two trips or a long stay, and none of the price pages read for this record states which it assumes.

What moves the number on these pages is the implant brand and the material of the bridge, in that order: acrylic hybrid at the bottom, zirconia at the top, with the same four screws underneath. Bone grafting and sinus lifting, the procedures the tilted-implant technique exists to avoid, are priced separately where they are needed.
