The largest source market in Southeast Asia, and the country whose departing patients built its neighbours' hospitals. Indonesians fly to Penang, Kuala Lumpur and Singapore in numbers their own government describes in the hundreds of thousands to a million a year, and the state has spent a decade trying to keep them home.

On 25 March 2022 the Deputy Minister of Health, Dante Saksono Harbuwono, put it in one sentence: "Every year, we lose 11.5 billion US dollars because many Indonesians, about 600 thousand to one million people, seek medical treatment abroad." The destinations he named were Malaysia, Singapore and Thailand.

That is a minister's estimate, given in a range that spans a factor of nearly two, and this record carries it as a statement rather than as a series. It is nonetheless the clearest official account of an outflow anywhere in Asia, and it is stated as a loss — which is how a source market's government sees this subject, and why so few of them fund a count.

The receiving end agrees. Indonesians were 75 per cent of Malaysia's foreign patients in 2008 and still 57 per cent in 2011, the share falling only because other nationalities grew.

Trade practice — the geography does most of the work. Penang is a forty-minute flight from Medan and has run a Sumatran patient trade for decades; Singapore takes the harder cases and the richer patients from Jakarta. The complaint that drives the journey is rarely price — it is waiting, referral and the way patients are spoken to, the same complaint a survey of Nepali travellers found.

Inference — an outflow described in dollars lost is a policy argument, not a measurement. The 11.5 billion figure has no published method attached to it, and nothing in this directory should be built on it.

How it works Trade practice

Trade practice — the trip is booked the way any regional trip is booked: a low-cost flight, a hospital's Indonesian-language desk, a deposit paid by transfer, and an interpreter who is often a Malaysian nurse who speaks Malay. Indonesian doctors' referrals travel informally, and the discharge summary comes home to a doctor who did not order the operation.
