Surgeons say Roux-en-Y, or mini bypass for the single-join version. Price lists put them 200 US dollars apart and do not always say which one the number belongs to.

The stomach is divided with staples to leave a small pouch at the top, and that pouch is joined to the lower part of the small intestine, so food passes most of the stomach and the first stretch of bowel without meeting them. The NHS puts the operation at around two hours. It restricts what can be eaten and reduces what is absorbed, and the second half of that sentence is what makes lifelong supplementation and blood monitoring a condition of having had it rather than an optional extra.

Root. bypass is plain English for a road built around a town, borrowed into surgery for a gut built around itself. Roux-en-Y names the shape of the join — two limbs of bowel meeting a third, drawn as a Y — after the Swiss surgeon César Roux, who described the reconstruction in the nineteenth century.
the mini or one-anastomosis bypass makes one join instead of two. It is a different operation with a different revision profile, and on a price list it looks like a discount. Trade practice

The bypass is the sleeve's more expensive neighbour on every package price list read for this record, and the destination of a sleeve that did not work. On one Tijuana page the two sit in a column with six other operations: a balloon at 1,880 US dollars, a sleeve at 4,580, a mini bypass at 5,780, a bypass at 5,980, a duodenal switch at 7,500. A patient reads that column as a menu of prices. It is a menu of different amounts of malabsorption.

Trade practice — the operation that reroutes the gut is the one whose aftercare cannot be skipped. Iron, B12, calcium, vitamin D and annual bloods are the standing requirement, and the deficiency that follows from missing them arrives years later, in a different country from the surgeon.

Inference — that timing is what makes the bypass the hardest bariatric operation to buy abroad well. The package is bought for a week; the operation's failure mode is measured in years; and none of the pages read for this record prices the correction of either a nutritional failure or a mechanical one.

Priced and bundled exactly like the sleeve — surgeon, theatre, anaesthetist, one to two nights on a ward, a hotel either side, transfers, a coordinator — and sold from the same pages, roughly 1,400 US dollars above it in Tijuana. What sits outside is also the same: flights, the companion, extra nights, take-home medication, lifelong vitamins, and any complication that declares itself after the flight home.

The legal and administrative facts attached to this operation are not prohibitions but payment rules. Whether a home insurer or a public system will pay for a complication of an operation it did not authorise, and whether it will fund a revision, is an insurance question rather than a criminal one, and it decides who bears the cost of the years the package does not cover.
