NHS England reported 7.3 million pathways waiting to start treatment at the end of July 2026 — an estimated 6.2 million unique patients — with 65.4% waiting up to eighteen weeks against a 92% standard and a median wait of 12.0 weeks. Ireland's National Treatment Purchase Fund counted 115,450 people waiting for an inpatient or day-case appointment at the end of May 2026, against 669,506 waiting for a first outpatient consultation. Three hours of theatre, a recovery room, two nights in a single room, the biopsy charge, and the surgeon, assistant and anaesthetist.

The exclusion list names one thing worth reading twice: "Special Medical supplies & Special Equipment used such as Ligasure Generator, Thunderbeat". Those are the energy devices a laparoscopic hysterectomy is routinely done with — the instruments that seal and divide vessels. Also outside: pre-operative tests, chest x-ray and ECG, outpatient charges, any extra theatre time, the difference if the room is upgraded, and take-home medication.

Inference — an operation priced at two nights with the sealing device excluded is priced the way the knee is priced with the implant excluded, and a reader comparing it with a state tariff is comparing a part with a whole. Ireland's own admitted-patient price for a hysterectomy for non-malignancy at minor complexity is EUR 13,282, which covers the admission entire — energy devices, tests and all.

General anaesthesia; for the laparoscopic route, three or four small incisions, the abdomen inflated with carbon dioxide, the uterus detached with an energy device and removed through the vagina, the vault closed. One to three nights, and six weeks before lifting anything heavy.

Where a state pays, the schedule prices the surgeon: Australia's Medicare Benefits Schedule sets item 35653, abdominal hysterectomy with or without removal of the tubes and ovaries, at a schedule fee of $807.60, with a 75% benefit of $605.70. Where a state prices the admission, as Ireland does for repayment purposes, the figure is sixteen times larger and counts different things.

As of 17 September 2026 the published Thai package price for a total laparoscopic hysterectomy is 265,000 baht for two nights, energy devices excluded; Ireland's admitted-patient price for the same class of operation is EUR 13,282 at minor complexity and EUR 19,412 at major; and the Australian schedule fee for the surgeon's service is $807.60.
