Surgeons say lap chole. Payer schedules say cholecystectomy and then split open from laparoscopic, because the two carry different prices and different stays.
The gallbladder is removed through four small incisions with a camera, or through a larger cut where the keyhole route fails. Done for gallstones — hardened deposits of digestive fluid, in the hospital page's own words — and for the pain and inflammation they cause. One night on the Bangkok package read for this record; a day case on the Irish tariff, which pays the same figure either way.

This is the operation where the two ways of writing a price line up most neatly, and they still disagree by a factor of three.
Bumrungrad in Bangkok lists laparoscopic cholecystectomy at 240,930 baht against a struck-through 267,700 under a label that reads Special Campaign Discount Common Procedures, package SUR04, prices stated valid until 31 December 2026. It buys an hour and a half of theatre, one night in a single room, the supplies and equipment used during surgery, the anaesthesia, the surgeon, assistant and anaesthetist, one day of ward rounds and the discharge summary. Robotic-assisted cholecystectomy on the same list is 349,500 baht, which is 45% more for the same organ. Bangkok Hospital publishes a robotic cholecystectomy package of its own.
Ireland prices the group rather than the technique: DRG H08B, laparoscopic cholecystectomy at minor complexity, EUR 9,867 — the same figure as an inpatient stay and as a day case, the guide excluding open cholecystectomy from that group and pricing it separately at EUR 16,206 as an inpatient and EUR 10,466 as a day case.
Australia pays the surgeon $768.90 under item 30443, cholecystectomy by any approach without cholangiogram, with a 75% benefit of $576.70.
Inference — a hospital charges for the route because the route costs it money; a health service pays for the outcome because it is buying a treated patient. The robot appears in the first kind of price list and not the second, and a reader comparing the two is watching two different accounting systems, not two different operations.
The queue is why any of it is read. Ireland's National Treatment Purchase Fund counted 115,450 people waiting for an inpatient or day-case appointment at the end of May 2026; NHS England's median wait across all pathways at the end of July 2026 was 12.0 weeks.
How it works Trade practice
General anaesthesia, the abdomen inflated with carbon dioxide, four ports, the cystic duct and artery clipped and divided, the gallbladder freed from the liver bed and drawn out through a port. An hour and a half of theatre on the package read for this record. Home the next day, back to desk work within a week or two, no lifting for a month.
Where the keyhole route has to be abandoned mid-operation, the bill changes category: the Irish schedule prices the open operation as a different group at two-thirds more, and the Thai package excludes "complex cases requiring emergency surgery or other conditions requiring equipment in excess of package". A conversion on the table is the commonest way a fixed price stops being fixed.
