In the trade Penang means George Town, and George Town means four hospitals within a kilometre and a half of each other on the north-east corner of the island.
The tightest cluster in this batch. Island Hospital stands at 308 Jalan Macalister in central George Town, a 600-bed hospital opened in 1996 and building toward a thousand; Gleneagles Penang is 600 metres north-west at 1 Jalan Pangkor in Pulau Tikus; Penang Adventist is on Jalan Burma between them, and Loh Guan Lye is a few streets east. A patient staying on Macalister Road can walk to two of them and take a five-minute taxi to the others.

Penang's hospitals were built for the island's own Chinese and Malay population and for the Straits trade, and the Indonesian patient arrived on top of that. A 2024 analysis reprinted by the Malaysian Investment Development Authority put Indonesia at 64.9 per cent of Malaysia's medical travellers and split national revenue between Kuala Lumpur at 41 per cent and Penang at 40.5 per cent; it attributes the figures to TA Securities rather than to a register. Earlier, the Malaysia Healthcare Travel Council's own numbers had Penang at 57 per cent of national revenue in 2009 and 66 per cent in 2010.
Those counts cover all registered patients holding a foreign passport, which is the council's own definition and includes everyone from a Medan family who flew for a scan to a Dutch resident who broke an ankle.
Trade practice — the flow is a ferry and a short flight rather than a long-haul journey. Medan is under an hour away by air, Aceh and Sumatra's north coast are the catchment, and Hokkien is spoken across the counter. What is sold is the screening, the scope, the cardiac work-up and the second opinion — high volume, low complexity, repeated — rather than the single spectacular operation.
Inference — that is why Penang takes almost as much national revenue as the capital from a city a fraction of the size, and why a tight harvest radius is the right one: the cluster genuinely is a few streets, and the volume comes from proximity rather than from reach.
How it works Trade practice
A Penang hospital quotes an estimate against a room category, and the screening packages are priced and published as bundles. The trade runs on repeat visits rather than on one arrival, and much of the booking is done through Indonesian agents and hospital representative offices in Medan and Jakarta rather than through the hospital's own website.
