A 1,991-bed public university hospital at Im Neuenheimer Feld in Heidelberg, treating over a million patients a year. It is an independent public-law institution with its own board, administratively and financially separate from Heidelberg University. Its international office runs an English-language site for patients from abroad, and that site publishes the payment terms rather than the prices.

The hospital dates its medical teaching to 1388 and its present form to a campus in the Neuenheimer Feld north of the Neckar. It holds the National Center for Tumor Diseases jointly with the German Cancer Research Center, and a heavy ion therapy facility whose 600-tonne rotating gantry is the piece of equipment the hospital is most often named for abroad. Newsweek's 2026 list placed it twelfth in the world.

What it publishes about money is worth reading closely, because it is the German answer to the package price and the closest thing in this batch to a German tariff.

A cost estimate exists only after the hospital's specialists have read the submitted records — "An overview of the expected treatment costs will be generated based on the treatment offer submitted by the expert" — and the hospital says plainly that the estimate may move. Then: "Once you've committed to receiving treatment at Heidelberg University Hospital, we ask that the entire sum be transferred to the account of Heidelberg University Hospital as given on the cost estimate." The appointment is confirmed on receipt of that transfer. After discharge the actual cost is worked out, and "if these costs are below the pre-payment, the balance will be refunded immediately" by the same route it arrived. Complications can require a further payment.

Inference — that is the opposite instrument from a fixed package. A package transfers the risk of the bill to the hospital and caps the patient's exposure at the advertised figure. A pre-payment against an estimate transfers the risk to the patient and reconciles afterwards. Neither is hidden; they are different contracts, and a reader comparing a Bangkok package with a Heidelberg estimate is comparing two different things.

Records first, decision second, estimate third, whole sum in advance, treatment, then reconciliation. Payment by bank transfer, by cash at the cashier's office on the ground floor of the Kopfklinik, or by card online or at the office.

No schedule of amounts is published for any procedure, so nothing here can be compared against a price list elsewhere without the hospital's own estimate in hand.
