The clinic calls it discharge. The system at home calls it a first presentation.

The interval between an operation abroad and the first clinician at home who sees the wound, during which nobody is responsible for the result. The American Medical Association's guiding principles, reproduced in the CDC's chapter, ask that "local follow-up care should be coordinated" before travel and that coverage include the cost of follow-up on return. The CDC also notes that "some overseas facilities and healthcare professionals charge substantial fees for follow-up care in addition to the base cost".

The gap is measurable from one side only, and what it looks like from that side is a paperwork failure. A Manchester breast unit reviewing twelve months of cosmetic surgery complications in 2022 and 2023 found 25 patients, 20 of whom had been operated on abroad. The precise date of surgery was recorded for 48% of them. The name of the clinic was recorded for 8%. The name of the surgeon was recorded for 0%. Seventy-two per cent presented within eight weeks of the operation.

That is not a study of infection or of technique. It is a study of what arrives: a patient, a complication, and no identifiable counterparty.

The General Medical Council's guidance for doctors offering cosmetic interventions, in force since 1 June 2016, sets out what the record is supposed to contain — a named contact for complications outside working hours, written information detailed enough "to enable another medical professional to take over the patient's care", and records organised so patients treated with a particular device can be identified if a safety concern arises. Those obligations bind a doctor on the UK register. The clinic that performed the operation is not on it.

Inference — the exposure transfers cleanly. The operating clinic's liability, such as it is, ends at the airport; the home system's begins at the emergency department door, without a contract, a referral or a fee.

The CDC asks clinicians to remind travellers to request copies of overseas records in English, and the American College of Surgeons goes further, asking that a complete set be obtained before returning home. Neither is enforceable at a distance. Where follow-up is offered by the operating facility it is usually by message or video, which cannot take a swab, remove a suture or drain a collection.

As of 16 September 2026 there is no published measure of how many people who travel for surgery attend any follow-up at all. The figures that exist are the recording rates in single-centre series of the patients who came back with a problem — a sample selected for having one.
