A clinic belt on the European side of Istanbul, thickest in Şişli — ten square kilometres holding 276,528 residents and the quarters of Nişantaşı, Teşvikiye, Pangaltı and Mecidiyeköy — and running north through Levent and Maslak. Şişli carries the Şişli Etfal state hospital, the American Hospital and the Saint James Armenian Hospital alongside clinic floors above the shops of Abdi İpekçi Avenue, which the district calls Turkey's most expensive street by lease price. Hair restoration is what the belt is known for abroad; the same square mile also sells rhinoplasty, dentistry, sleeve gastrectomy and IVF. Every facility and every agency trading with foreign patients must hold a Ministry of Health authorisation certificate.

Türkiye put the trade on a licence in 2017. The Uluslararası Sağlık Turizmi ve Turistin Sağlığı Hakkında Yönetmelik — the ministry's regulation on international health travel and on the health of visitors — appeared in the Resmî Gazete on 13 July 2017, issue 30123, and was replaced by a regulation of the same name on 26 April 2025, issue 32882, whose MADDE 17 repealed it. Its Article 5 makes an international health tourism authorisation certificate compulsory for both the treating facility and the intermediary agency; the Ministry halts the international work of anyone trading without one. Facilities already in the business were given a year to comply.

Trade practice — what the licence regulated was already a mature export. The city's hair clinics sell a fixed bundle — airport pickup, a hotel, the procedure, a night, a first wash — and compete on graft count and on price rather than on a surgeon's name.

The surgical society has spent a decade objecting to the staffing. The International Society of Hair Restoration Surgery issued a release on 18 May 2016 naming Turkey and warning that unqualified technicians perform the surgery illegally, and reporting Turkish technician groups working in Germany, the Netherlands and Belgium under the cover of licensed doctors. Its standing Black Market campaign, launched in November 2019, states that unlicensed personnel worldwide carry out substantial medical parts of hair restoration surgery, and its Operation Restore programme repairs the results.

Inference — the two facts sit together without contradicting each other. A national licence certifies a facility and an agency. Who holds the punch inside the room on the day is a separate question, and it is the one the society keeps asking.

Trade practice — the Istanbul form is the package: a flight met at the airport, a hotel in Şişli or Mecidiyeköy, a consultation the morning of surgery, the procedure the same day, one or two nights, a wash and a discharge folder, and a flight home inside a week. Payment is a deposit to hold the date and the balance on arrival. The graft count is the price variable, and the number quoted before travel is an estimate the clinic revises when it sees the head. Aftercare is a messaging app: the follow-up is remote by design, and a revision is a second flight.

As of 17 September 2026 the instrument is the 2025 regulation, not the 2017 one that most writing on this city still cites; the authorisation certificate is the thing to ask a clinic for by name, and the Ministry's own register is where it is checked. Istanbul's population was 15,754,053 at the end of 2025. This record carries no count of how many people fly in for surgery: the figures in circulation count arrivals who state a health purpose, which is not the same as operations. Hair transplant units answer to a rule of their own as well — the Saç Ekimi Birimleri Yönetmeliği of 6 May 2023, Resmî Gazete 32182 — which reserves the channel-opening stage to certified physicians and forbids more than one patient in a room at a time. The Ministry's four published lists of authorised facilities, stamped 20 August 2026, run to 542 hospitals, 135 medical centres, 2,468 physician offices and 1,581 other facilities; the Ministry prints no total.
